Job summary

Tidewell Hospice, part of Empath Health, is seeking an RN PRN Case Manager to coordinate patient and family care across home, facility, and assisted-living settings in southwest Florida. You will review plans, physician orders, and medications while educating families about illness and end-of-life care.

Minimum one year in acute care with hospice experience preferred; Florida RN license required; BSN/MSN preferred. Join a mission-driven team delivering Full Life Care across Florida.

Qualifications

• Graduate of an approved nursing program.
• Current Florida RN license.
• Minimum one year experience in an acute-care hospital.
• Hospice nursing experience preferred.
• BSN/MSN preferred.

Responsibilities

• Create and maintain the plan of care as the coordinator for patient and family interactions.
• Review and update the care plan, physician orders, pharmacy profile and other records.
• Educate patients, family members and caregivers about disease progression, palliative options, and safety practices.
• Provide nursing care in home, long-term care, or assisted-living settings.
• Deliver compassionate nursing care in emotionally intense situations.
