Job summary

Empath Health’s Tidewell Hospice is seeking a dedicated RN to support patients in Lakeland, FL. You will provide after-hours clinical and emotional care, guided by the Triage team, in home, facility, or hospital settings.

You’ll assess patients under physician supervision, manage symptoms, and educate families to improve quality of life. BSN/MSN preferred; Florida RN license and CPR are required; CHPN encouraged within three years.

Qualifications

• Graduate of approved nursing program with current, unrestricted Florida RN license.
• CPR certification required and current.
• Minimum one year of acute-care hospital experience providing direct patient care; hospice RN experience preferred.
• CHPN certification encouraged within three years of employment.
• BSN or MSN preferred.
• Equivalents may be considered with appropriate education and experience.

Responsibilities

• Visit patients and provide clinical, physical, educational and emotional support after-hours as directed by the Triage team.
• Under physician supervision, perform patient assessment and manage discomfort with palliative nursing skills.
• Relieve symptoms and maximize comfort to improve patient/family quality of life.
• Attend patient deaths and provide bereavement support to patient caregivers and family; perform post-mortem care as needed.
• Document services accurately in the EMR to reflect the care plan and outcomes.
• Review and update the Plan of Care, physician orders, and MAR/Pharmacy records to meet patient/family needs.
