Qualifications

• Graduation from an accredited school of nursing and Michigan RN licensure.
• Minimum of one year Hospice RN experience.
• Hospice certification preferred.
• Must hold a valid driver's license and have acceptable driving record.
• Ability to adapt policies for Hospice patients and families.

Responsibilities

• Assesses and identifies patient needs after hours in hospice care.
• Coordinates with the Interdisciplinary Team to provide patient/family care.
• Maintains continuity of care by ensuring patient needs are met.
• May perform triage on call after hours or provide home visits as needed.
