Job summary

Henry Ford Health in Detroit is seeking a Registered Nurse - Hospice - After Hours to assess and address patient needs after hours within the hospice program, coordinating with the interdisciplinary team.

The role requires Michigan RN licensure and hospice RN experience; a driver's license; flexibility; hospice certification preferred.

Full-time midnight shift with after-hours responsibilities, contributing to compassionate end-of-life care and support for patients and families.

Qualifications

• Must graduate from an accredited school of nursing and hold a current Michigan RN license.
• Minimum of one year Hospice RN experience.
• Understanding of hospice care concepts and knowledge of patient/family responses to hospice and terminal illness.
• Valid driver's license and acceptable driving record.
• Hospice certification preferred.

Responsibilities

• Assess and identify patient needs after hours according to hospice care.
• Coordinate with Interdisciplinary Team to deliver patient and family care.
• Maintain continuity of care by ensuring patient needs are met.
• May perform on-call hospice triage after hours or provide home visits as needed.
