Job summary

3HC Home Health and Hospice in Smithfield, NC seeks a dedicated Hospice Nurse, RN (Admissions) for a full-time role. The Nurse coordinates hospice services in assigned counties, ensuring compassionate, high-quality care in alignment with agency goals and policies.

The ideal candidate is a NC-licensed RN with leadership qualities, strong communication, and experience with terminally ill patients. Must maintain CPR certification and a valid driver's license for home visits and on-call coverage.

Qualifications

• Registered Nurse licensed to practice in North Carolina.
• Minimum 1 year experience as a professional nurse in an acute care setting preferred.
• Evidence of leadership qualities.
• Previous experience with terminally ill patients preferred.
• Ability to communicate well with patients, family, staff, physicians, and the public.
• Maintain current CPR certification (BCLS).
• Must be dependable and able to interact tactfully with patients, families, staff, providers, and the public.
• Competent to perform delegated tasks to support the care plan.

Responsibilities

• Abides by 3HC's Compliance Program and Code of Ethics.
• Creates positive experiences for internal and external customers.
• Provides comfort and coordinates hospice care per physician plan of care.
• Maintains electronic documentation in EMR within 48 hours.
• Communicates with physician/Hospice Medical Director about care changes.
• Supervises LPNs and paraprofessionals per CMS/Hospice guidelines.
• Rotates on-call and back-up responsibilities; covers visits as needed.
• On-call responsibilities for base office and special nurse roles as needed.
