Job summary

Ohio's Hospice is seeking a compassionate RN Admission Care Liaison to join the South Region, serving the greater Miami Valley. This full-time, field-based role requires travel to patient homes, healthcare facilities, and other care settings, with a 10:00 a.m. to 10:30 p.m.

Qualifications

Graduate of an accredited nursing program.
Active, unrestricted RN license in Ohio.
Current CPR certification.
Strong computer skills and EMR proficiency.
Valid driver’s license and reliable transportation.

Responsibilities

Complete hospice admissions accurately and compassionately.
Conduct comprehensive patient assessments and determine appropriate hospice services.
Educate patients and families about hospice philosophy, services and options.
Coordinate care with patients, families, referral sources and healthcare providers.
Establish and maintain relationships with referral sources and partners.
Document assessments and interactions in the EMR at the point of care.
Support a 75% admission conversion rate.
Ensure compliance with hospice regulations and standards.
Advocate for patients and families during the transition to hospice.
