Job summary

Ohio's Hospice, Inc. is seeking an RN Admission Care Liaison to join the South Region. The role involves completing hospice admissions, conducting nursing assessments, coordinating services with patients, families, and providers, and facilitating a smooth transition to hospice care.

The ideal candidate will demonstrate clinical judgment, strong communication, patient advocacy, and a commitment to hospice care. Travel across the service area is required with day and night driving as needed.

Qualifications

• Graduate of an accredited nursing program.
• Active, unrestricted RN license in Ohio.
• Current CPR certification.
• Strong computer skills and proficiency with EMR.
• Valid driver’s license and reliable transportation.
• Automobile insurance meeting state minimums.
• Ability to travel throughout the service area, including night driving and adverse weather.

Responsibilities

• Complete hospice admissions accurately and compassionately.
• Conduct comprehensive patient assessments and determine services.
• Educate patients and families about hospice philosophy and options.
• Coordinate care with families, referral sources, providers, and partners.
• Maintain professional relationships with referral sources and partners.
• Document assessments and services in the EMR at point of care.
• Meet performance expectations, including a 75% admission conversion rate.
• Ensure compliance with hospice regulations and policies.
• Advocate for patients and families during transition to hospice.
