Job summary

Ohio’s Hospice, Inc. in Newark, Ohio is seeking a Part-Time Admission Care Liaison (Registered Nurse) to guide patients and families through the admission process and explain hospice options.

The role involves coordinating with physicians and interdisciplinary teammates across Central Ohio settings. The RN will perform comprehensive assessments, communicate patient status, and support transitions from hospital to hospice care while maintaining compassionate, patient-centered care.

Qualifications

• Nursing degree or diploma from an accredited program.
• Current Ohio Registered Nurse license in good standing.
• CPR certification.
• Proficient computer skills for clinical documentation.
• Valid Ohio driver’s license and auto insurance.
• Ability to travel throughout Central Ohio service area.
• Successful completion of a background check and drug screen.

Responsibilities

• Complete comprehensive nursing assessments to evaluate patients’ clinical needs and hospice eligibility.
• Meet with patients and families in hospitals, long-term care facilities, and home settings.
• Explain hospice services, eligibility, and available resources to help informed decisions.
• Coordinate admissions and individualized plans of care with the interdisciplinary team.
• Document clinical findings and patient status for eligibility determination.
• Coordinate transitions from hospitals to hospice services.
• Collaborate with hospital and facility staff to promote continuity of care.
• Provide ongoing assessments and symptom management as needed.
• Participate in quality improvement and staff development initiatives.
• Assist with staff orientation and mentoring.

Education

Bachelor's/Associate degree in Nursing

Ohio RN license

CPR certification

Computer skills for clinical documentation

Valid Ohio driver’s license

Able to travel across Central Ohio
