Job summary

Ohio’s Hospice is seeking a compassionate RN Admission Care Liaison to join the South Region. You will complete hospice admissions, conduct nursing assessments, and coordinate services for patients and families, with field-based travel throughout the service area. The role requires strong clinical judgment, excellent communication, and a commitment to compassionate end-of-life care.

You will work 10:00 a.m. – 10:30 p.m.

Qualifications

• Active, unrestricted RN license in Ohio.
• Current CPR certification.
• Strong computer skills and proficiency with electronic medical records.
• Valid driver’s license, reliable transportation, and a safe driving record.
• Ability to travel independently throughout the service area, including driving at night and in adverse weather conditions.

Responsibilities

• Complete hospice admissions accurately, efficiently, and compassionately.
• Conduct comprehensive patient assessments and determine appropriate hospice services based on patient needs and eligibility criteria.
• Educate patients and families about hospice philosophy, services, care options, and available resources.
• Coordinate care with patients, families, referral sources, healthcare providers, and community partners.
• Establish and maintain positive professional relationships with referral sources and healthcare partners.
• Complete timely, accurate documentation of assessments, patient interactions, and services in the electronic medical record at the point of care.
• Meet organizational performance expectations, including a 75% admission conversion rate.
• Ensure compliance with applicable hospice regulations, organizational policies, and clinical standards.
• Advocate for patients and families while supporting a compassionate, seamless transition to hospice services.
