Job summary

Graham Healthcare Group in Park Ridge, IL is seeking a Registered Nurse for Hospice care, delivering compassionate home health services to patients in their homes within the Franklin Park area. This full-time staff RN role includes 40 hours per week with 8-hour shifts and a $10,000 first-year success bonus.

Qualifications

• Active RN license in the state you work.
• Two years RN experience and hospice experience preferred.
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care at patients' homes, collaborating with an interdisciplinary team.
• Perform comprehensive patient assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed and cared for.
• Maintain thorough, compliant EMR documentation.
• Travel to patient homes within assigned territory.
• Work independently, but never alone.
