Graham Healthcare Group is seeking a Registered Nurse (RN) Hospice to provide end-of-life care in patients' homes in Morton Grove, Illinois. This 40-hour-per-week staff position offers meaningful patient relationships and autonomy in a field-based role.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Driver’s license required; travel ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning.
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
