Interim HealthCare Hospice in Middleton, WI is hiring a Hospice Registered Nurse (RN). You will provide compassionate, home-based care to patients with limited life expectancy, collaborating with a diverse team to implement the plan of care.

This role emphasizes comfort, quality of life, and family support. The ideal candidate is a graduate of an accredited nursing program with an active Wisconsin RN license and at least 1 year of nursing experience, preferably in hospice or home health, plus

Graduate of an accredited nursing program and active RN license in Wisconsin.
Minimum of 1 year of nursing experience, ideally in hospice or home healthcare.
CPR certification required.
Knowledge of state and federal home health regulations.
Clinical proficiency, strong communication skills and compassion.

Responsibilities

Provide home-based care to patients with six months or less to live, focused on quality of life.
Collaborate with an interdisciplinary team to carry out the patient’s plan of care.
Perform patient assessments, monitor vitals, manage pain, and administer prescribed medications.
Ensure accessibility of medical equipment, oxygen, medications and supplies.
Educate families on prognosis, care plan changes and hospice goals.
Adopt a holistic approach engaging mind, body, spirit, and family.
