Job summary

Interim HealthCare of Manhattan is seeking a Registered Nurse (RN) for a 50% Hospice On-Call role in the Manhattan, Kansas area. You will provide home-based, patient-centered care as part of an interdisciplinary team, focused on improving quality of life for patients with limited life expectancy.

The position requires an active KS RN license, and a track record or strong interest in hospice care. You’ll enjoy flexible assignments, autonomy, and opportunities for ongoing training and CEUs in a

Qualifications

Graduate of an accredited school of nursing.
Licensed to practice as an RN in Kansas (active and in good standing).
Twelve months of RN experience within last five years, or current clinical knowledge per DHCS.
If a new graduate, 1200 verified hours in last 12 months or completion of Preceptor Program.
One year hospice care experience preferred.

Responsibilities

Provide home-based care to patients with six months or less to live, focusing on quality of life.
Work with interdisciplinary team (physician, LPN, CNA, HHA, social worker, chaplain, volunteers) to carry out the plan of care.
Perform patient assessments, check vitals, evaluate breathing, monitor pain, and administer prescribed medications and treatments.
Ensure accessibility of medical equipment, oxygen, medications and other supplies.
Educate family on prognosis, care plan changes, and hospice goals.
Take a holistic approach to care involving patient, family, mind, body and spirit.
