Job summary

Cherish Hospice is seeking an experienced RN Case Manager to lead compassionate, patient‑centered care in the Canton area. You will coordinate a multidisciplinary team, build personalized care plans, and advocate for each patient’s preferences and quality of life.

This full‑time role offers flexible scheduling, meaningful work, and support for families, with travel to patients’ homes and facilities. An active RN license, valid driver’s license, and hospice experience are required; BLS preferred.

Qualifications

• Active RN license and valid driver’s license required.
• Minimum 1 year hospice experience preferred.
• BLS certification preferred; hospice/palliative background beneficial.

Responsibilities

• Lead a multidisciplinary team to create personalized care experiences.
• Advocate for each patient’s needs, preferences, and wishes while supporting families.
• Build meaningful connections, provide counseling and emotional support.
• Develop and implement care plans prioritizing quality of life and comfort.
• Document each patient’s journey and progress in the chart.
