Qualifications

• Active RN license and valid driver's license required.
• 1+ years hospice experience preferred.
• BLS Certification preferred.
• Hospice and Palliative Medicine background preferred.

Responsibilities

• Lead multidisciplinary team including LPNs, Aides, Social Workers and Chaplains to create personalized care plans.
• Advocate for each patient’s needs, preferences and wishes while supporting families.
• Provide counseling and emotional support to patients and loved ones when needed.
• Develop and implement care plans that prioritize quality of life and comfort.
• Document patient journeys and progress to tell their story accurately.
