Job summary

Visiting Nurse Association Health Group in Monmouth County is seeking a Registered Nurse – Hospice to join our per diem hospice team. You will assess patient needs, manage symptoms, develop individualized care plans, and support families through the end-of-life journey with compassion and dignity.

Responsibilities include completing orientation, assessing patients, educating families, collaborating with clinicians and external providers, and updating care plans as goals evolve.

Qualifications

• RN with Bachelor’s degree in Nursing.
• Certification in Hospice Palliative Care Nurse and Critical Care preferred.
• 1+ year of supervised clinical experience as a registered nurse.
• Excellent communication skills and strong computer literacy.

Working Conditions/Physical Demand

Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license
