Job summary

VNA Health Group in New Jersey seeks a Per Diem Staff Nurse Hospice to join our Monmouth County team. You will assess patient needs, manage symptoms, and develop individualized care plans to support patients and families at end of life.

The role requires an RN with a BSN, strong communication and computer skills, and the ability to collaborate with clinicians and external providers. Certification in Hospice Palliative Care is preferred.

Qualifications

• RN with Bachelor's degree in Nursing.
• 1+ year of supervised clinical RN experience.
• Hospice Palliative Care Certification and Critical Care Certification preferred.
• Excellent communication, computer, organizational, and interpersonal skills.

Responsibilities

• Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
• Assess patients with advanced illnesses and create individualized hospice care plans.
