Job summary

HopeHealth is seeking a Hospice Field LPN for its affiliate HopeHealth Hospice & Palliative Care in Providence, RI. The Response Team LPN responds to urgent patient needs with strong assessment skills and the ability to quickly determine the best course of action.

Hours are 24 per week, 8:00am–4:30pm, with a rotating weekly schedule: Week 1 Sun, Tue and Fri; Week 2 Mon, Thu and Sat plus one major and one minor holiday. You will assess, plan, educate, and coordinate with the care team.

Qualifications

• LPN license is required; BSN preferred.
• Licensed as a Practical Nurse in Rhode Island and Massachusetts.
• Minimum of 1–3 years of clinical experience.
• Excellent assessment, clinical and communication skills.
• Bilingual preferred but not required.

Responsibilities

• Initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s needs and activities of daily living.
• Implements the individualized plan of care and revises it as needed; plans for discharge/transfer.
• Educates patient/family/caregiver on disease process and self-care techniques; provides instructions as ordered by the physician.
• Initiates preventive and rehabilitative nursing procedures.
• Collaborates with the interdisciplinary team to develop and revise the care plan.
• Notifies physician of changes and updates the plan of care as needed.
• Coordinates scope and frequency of services based on acuity and needs.
• Coordinates care with family/caregiver; addresses psychosocial, spiritual and emotional needs.
