Job summary

Agrace seeks a Resource Registered Nurse (Resource RN) in Baraboo, WI for 0.8 FTE (32 hours/week). The role covers day shifts on weekdays with occasional weekend/holiday rotations, and involves assessments, plan of care updates, triage, and care delivery across homecare teams.

The RN must hold an active WI license, have a driver’s license, and be able to work in a patient-centered hospice setting. A sign-on bonus of $6,000 is offered.

Qualifications

• ADN required; BSN preferred.
• Current, unrestricted RN license in Wisconsin.
• Valid driver’s license and access to reliable vehicle.
• Experience with home health or hospice care is a plus.

Responsibilities

• Assess patients and families and document findings in the plan of care.
• Perform patient visits, provide interventions and pain management.
• Develop and revise care plans with the IDT and family.
