Job summary

Superior Hospice, part of the Home Health Care, Inc. family, seeks a compassionate RN to deliver hospice care in patients' homes, assisted living communities, memory care, and skilled nursing facilities near Little Canada, MN.

You will assess symptoms, provide education on medications, wound care, and comfort-focused interventions, and document care in the EMR. Collaboration with the interdisciplinary team is essential to tailor care for each patient.

Qualifications

• Active Minnesota RN license required.
• Hospice, home health, oncology, ICU, or palliative care experience preferred.
• Excellent communication and relationship-building skills.
• Ability to work independently with a field-based schedule.
• Reliable transportation and a valid driver’s license.

Responsibilities

• Provide scheduled and PRN hospice visits in patients' homes, assisted living communities, memory care, and skilled nursing facilities.
• Assess patient needs and provide symptom management, medication education, wound care, and comfort-focused nursing interventions.
• Educate and support patients and families throughout their hospice journey.
• Collaborate with the interdisciplinary team to ensure individualized, compassionate care.
• Complete timely and accurate EMR documentation.
