Job summary

Lorian Health in San Diego is seeking a compassionate RN to provide patient-centered hospice nursing care in the home and community settings. This role emphasizes comprehensive assessments, care planning, symptom management, and coordination with an interdisciplinary team.

You will collaborate with physicians, educate families, maintain accurate documentation, and support end-of-life care with dignity and empathy. On-call rotation is part of the schedule, with a competitive hourly wage.

Qualifications

• Graduate of an accredited school of professional nursing; BSN preferred.
• Currently licensed to practice as an RN in the state of practice.
• If IV or ET certified, certified and recognized by the state(s) where currently practicing as an RN.
• Minimum of one year of recent Home Health or Hospice experience (within last 3 years).
• Possess and maintain a valid driver’s license.

Responsibilities

• Provide compassionate, patient-centered nursing care to hospice patients and families in home/community settings.
• Complete assessments to identify patient and family needs.
• Develop, implement, and update hospice and palliative care plans.
• Assess and manage pain, symptoms, medications, and treatments per plan of care.
• Serve as Case Manager and coordinate care with the interdisciplinary team.
• Educate patients and families on medications, symptoms, disease progression, and comfort-focused care.
• Communicate changes in patient condition to physicians and team.
• Maintain accurate clinical documentation and care continuity.
• Support patients and families with physical, emotional, and end-of-life needs.
• Coordinate equipment, supplies, and services to support quality care.
• Participate in team meetings, quality improvement, and continuing education.

What You Will Do

• Provide compassionate, patient-centered nursing care to hospice patients and their families in the home and community setting.
• Complete comprehensive and ongoing assessments to identify patient and family needs.
• Develop, implement, and regularly update individualized hospice and palliative care plans.
• Assess and manage pain, symptoms, medications, and treatments in accordance with the patient’s plan of care.
• Serve as a Case Manager when assigned and coordinate care across the patient’s interdisciplinary hospice team.
• Educate patients and families on medications, symptom management, disease progression, and comfort-focused care.
• Communicate changes in patient condition and care needs with physicians and members of the interdisciplinary team.
• Maintain accurate, timely clinical documentation and ensure continuity of care.
• Support patients and families with their physical, emotional, and end-of-life care needs.
• Coordinate necessary equipment, supplies, and services to support quality patient care.
• Participate in interdisciplinary team meetings, quality improvement activities, and continuing education.

Required Qualifications

• Is a graduate of an accredited school of professional nursing, BSN preferred.

• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Possess and maintains a valid driver’s license.

Range: $53-$57 per hour, commensurate with experience.
