Job summary

Lorian Health in San Diego, CA is seeking an experienced Registered Nurse to join our home health team. You will assess patients, update care plans, manage symptoms, and educate families about care and medications.

A valid RN license and CPR certification are required, with a driver’s license and insured vehicle to serve patients in the region. Preferred candidates have 2+ years of RN experience or BSN, and the ability to lift and position patients.

Qualifications

• Must hold a valid RN license and CPR certification.
• 1+ year of RN experience or a BSN is preferred.
• Ability to lift/position patients and stand/walk frequently; reliable vehicle and driver's license required.

Responsibilities

• Complete patient assessments and update care plans.
• Provide symptom and pain management.
• Educate patients and families on care and medications.
• Coordinate with physicians and interdisciplinary team.
• Document timely, accurate clinical notes.
• Manage assigned caseload and ensure a safe care environment.
• Participate in on‑call rotation.
