About the Role

The RN position at Lorian offers a challenging and rewarding career opportunity for a skilled and compassionate nurse to provide high-quality patient care in a dynamic and supportive environment.

Responsibilities

• Assesses and evaluates patient/client status by:
  • Reviewing and implementing the Plan of Care (POC)
  • Regularly re-evaluating patient/client needs
  • Revising the POC as necessary
• Administers medications and treatments as prescribed by the physician.
• Initiates and applies appropriate preventative, therapeutic, and rehabilitative nursing procedures and techniques and maintains communication regarding patient/client care with the physician and supervisory personnel.
• Regularly observes the patient/client for changes in condition, discusses changes with the patient/client or family/caregiver and reports changes, as necessary, to the attending physician.
• Assists the patient/client with activities of daily living and facilitates the patient/clients effort toward self-sufficiency and independence.
• Teaches the patient/client and family/caregiver self‑care techniques as appropriate.
• Provides medication, diet, and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/clients and families/caregivers.
• Provides and maintains a safe environment for the patient/client.

Qualifications

• Currently licensed to practice as a RN in the state of practice.
• If IV or ET certified, is currently certified and recognized by the state(s) where currently practicing as an RN.
• Possesses excellent observation, good nursing judgment, and good oral and written communication skills.
• Possesses and maintains good physical stamina and mental health. Presents pre‑employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statutes.
• Possesses and maintains current CPR certification.
• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $80-170, visit commensurate with experience and depending on type of visit.
