Job summary

The Elizabeth Hospice is seeking an RN Case Manager to provide professional nursing care for adults, pediatrics and perinatal patients. Working with the IDT, you will assess, develop, implement and evaluate the plan of care and coordinate communication with internal and external customers.

The role supports the organization’s mission to enhance quality of life at end of life, with a Monday–Friday schedule, two weekend shifts monthly, and on-call shifts.

Qualifications

  • Associates degree plus 1 year of nursing experience or BSN from ACEN-accredited program.
  • Current CA RN license.
  • Current CA driver’s license with proof of auto insurance.
  • Current CPR certification and negative TB skin test.
  • Hospice nurse certification preferred.
  • Pediatric experience preferred.
  • Spanish, Farsi or Tagalog speaking preferred.
  • Computer skills (word processing) and basic office equipment.
  • Willingness to drive in various conditions and enter homes with pets.

Responsibilities

  • Oversees implementation of plan and updates as needed.
  • Directs care provided by LVN and HA per the plan of care.
  • Supervises services per TEH policy and regulations.
  • Communicates changes to IDT and primary MD.
  • Maintains timely patient documentation in electronic charting.
  • Implements hospice policies and regulatory standards.
  • Provides health care instruction to patient and family.
  • Orients and trains assigned personnel.
  • Supports 24/7 on-call coverage and census planning.
  • Performs admission assessment when necessary.

Description

The RN Case Manager is primarily responsible for providing professional nursing care to assigned patients, which includes adults, pediatric and perinatal. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. Ensures appropriate communication and plan of care coordination to all internal/external customers. Responsible for carrying out the mission, vision, values and commitment to quality of The Elizabeth Hospice.

The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego County and south Riverside County with multiple hospice office locations.

Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m. Two weekend shifts required monthly.

On Call: Two on-call shifts required monthly.

Salary: $45.00-$61.00 hourly. Compensation is determined by years of licensure.

Work Location: North San Diego County CA

Travel: Travel is required for patient visits.

Requirements
Responsibilities

1. Develops written plan of care in coordination with an interdisciplinary Team (IDT).

2. Oversees implementation of plan, evaluates and changes plan as necessary to meet patient/family needs.

3. Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care.

4. Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements.

5. Communicates all changes to IDT and primary MD.

6. Performs job duties in clinical areas as stated in clinical competency checklist, policies/procedures, standards of care, and critical competencies based on Scope of Practice.

7. Maintains timely patient documentation using “point of care” electronic charting.

8. Implements and maintains established hospice policies and procedures as well as Standards of Care and regulatory requirements.

9. Provides health care instruction to patient and family members.

10. Orients, instructs, and trains assigned personnel.

11. Participates in providing 24 hour, seven days a week coverage to patients/families (on-call and census fluctuation).

12. Performs admission assessment and admission when necessary.

13. Adhering to the E-way standards is a requirement in performing all job duties.

Qualifications:

1. Must have Associates Degree plus one (1) year of experience as a professional nurse within the past three (3) years or baccalaureate degree in nursing from a program accredited by the Accreditation Commission for Education in Nursing (ACEN) or current accreditation program) required.

2. Professional nursing experience preferred in hospice, oncology, emergency room, pain clinic or medical-surgical nursing.

3. Current CA RN license.

4. Current CA driver’s license with proof of auto insurance.

5. Current CPR certification and negative TB skin test.

6. Prefer certification as hospice nurse.

7. Pediatric experience preferred.

8. Spanish, Farsi or Tagalog speaking preferred.

9. Computer skills (word processing), use of adding machine, calculator, fax, photocopier.

10. Because this is a field position, must be able to drive in various conditions, including but not limited to winding/steep roads, night driving, and rural areas, and be able to enter homes with pets.
