Job summary

VIA Health Partners is seeking an experienced registered nurse to coordinate and deliver hospice and palliative care for patients in the Charlotte, NC area. You will conduct nursing assessments, develop and update care plans, educate patients and families, and collaborate with the IDG to ensure continuity of care.

Must hold an active RN license; Diploma or ADN required, BSN preferred. CPR is required; CHPN is a plus.

Qualifications

• Active RN license in good standing.
• Diploma or Associate degree in nursing required; BSN preferred.
• CPR certification required; CHPN preferred.
• Experience in inpatient units and hospice/palliative care preferred.

Responsibilities

• Coordinate and deliver patient/caregiver care.
• Perform nursing assessments and update care plans as needed.
• Educate patients and families; promote independence where possible.
• Communicate with IDG and other health professionals for continuity of care.
• Maintain accurate clinical documentation per guidelines.
• Comply with infection control and universal precautions.

Keeps supervisor apprised of any complex patient/caregiver issues.Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices.Interdisciplinary GroupAttends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.Assumes responsibility for establishing and maintaining effective working relationships with team members.Identifies specific patient/caregiver needs and make referral to appropriate team members.DocumentationEnsures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.Professional and Personal AccountabilityDemonstrates competency in knowledge and skills.Ensures that VIA Health Partners professional reputation is maintained and projected.Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Able to plan workday while remaining able to respond appropriately to crisis.May perform other duties as required(IPU) Cleans patient rooms as required per patient room cleaning procedure. Ensures all areas of facility are clean and appropriately stocked with supplies. Keeps the family room clean throughout the day. Ensures proper chemicals are used for cleaning different surfaces. Ensures protective equipment is used when working with chemicals, i.e., gloves, glasses.Minimum QualificationsA Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.Current CPR required; CHPN preferred.Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.Computer proficiency is required.
