Qualifications

  • Diploma or associate degree in nursing; bachelor's degree preferred.
  • Current RN license (compact, multistate) required.
  • CPR certification; CHPN preferred.
  • Minimum 2 years related patient care experience, esp. life-limiting illnesses.
  • Computer proficiency required.

Responsibilities

  • Evaluates and reports patient/family needs and symptoms to palliative care providers to guide care decisions.
  • Liaises with primary care physicians and palliative care providers regarding orders and care plans.
  • Educates patient and family about palliative care and the plan of care.
  • Coordinates community resources with the palliative care team and social worker.
  • Applies infection control principles and universal precautions per guidelines.
  • Attends and may facilitate interdisciplinary group conferences; maintains professional relationships with team.

Essential Functions
Delivery of Patient Care
  • Evaluates/re-evaluates patient/family needs, symptoms, and condition in order to report accurately to the Palliative Care providers so appropriate decisions may be made regarding patient/family care.
  • Serves as the liaison for the primary care physician and palliative care providers regarding physicians' orders for the care plan; responsible for continued communication with physicians and other health care personnel concerning changes in medical treatment and all other aspects of patient care.
  • Provides education and counsel to the patient/family regarding palliative care and their plan of care.
  • Coordinates community resources for patients' needs as assessed by the palliative care team in conjunction with the social worker.
  • Applies principles of infection control. Follows guidelines for universal precautions in accordance with accepted professional practices.
Interdisciplinary Group
  • Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.
  • Assumes responsibility for establishing and maintaining effective working relationships with team members.
  • Identifies specific patient/caregiver needs and make referral to appropriate team members.
  • Assists the administrative assistant with managing the referral process, provider assignments, provider.
Documentation
  • Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.
  • Reviews and updates pain contracts for each palliative care provider on a weekly basis.
Professional and Personal Accountability
  • Demonstrates competency in knowledge and skills.
  • Ensures that VIA Health Partners professional reputation is maintained and projected.
  • Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.
  • Maintains current professional licensure, CPR certification and CEU requirements.
  • Maintain productive and professional relations with external agencies and healthcare providers.
  • Exhibits time management skills. Able to plan workday while remaining able to respond appropriately to crisis.

May perform other duties as required.

Qualifications
Minimum Qualifications
  • A Diploma or associate degree in nursing is required; bachelor's degree preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
  • The Nurse Licensure Compact (NLC) allows a registered nurse (RN) and licensed practical/vocational nurse (LPN/LVN) to have one multistate license in a primary state of residency (the home state) and to practice in other compact states (remote states), while subject to each state’s practice laws and discipline.
  • Current CPR required; CHPN preferred.
  • Minimum 2-year related experience. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics, oncology; home health care; medical surgical.
  • Computer proficiency is required.
