Job summary

PruittHealth Hospice is seeking a licensed Registered Nurse Case Manager to deliver hospice nursing care in patients' homes or care facilities. You will perform evaluations, plan and implement care, and document medical records according to policy.

Qualifications

• Current, active, and unrestricted RN license in the state of practice.
• Experience in hospice or palliative care preferred.
• Ability to perform evaluations and care planning.
• Strong documentation skills and care coordination.

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
