Job summary

Constellation Health Services is seeking a compassionate Registered Nurse to deliver hospice care in patients' homes, assisted living facilities, and skilled nursing facilities in Orange, CT. This PRN/Per Diem role supports families and collaborates with an interdisciplinary team to ensure high-quality, respectful care.

The ideal candidate is a graduate of an accredited RN program, holds an active CT RN license, and has 1–2 years of RN experience (hospice preferred).

Qualifications

• Graduate of an accredited RN program.
• Active RN license in the state of employment.
• 1–2 years RN experience required; hospice strongly desired.

Responsibilities

• Provides skilled nursing care including administration of medications and treatments.
• Completes comprehensive assessments and coordinates plan of care with all disciplines.
• Documents all nursing care and updates the team through the required system.
• Participates in interdisciplinary team meetings and communicates patient status.
• Weekend and holiday coverage as needed.
