Constellation Health Services is seeking a compassionate Hospice Registered Nurse Case Manager to support patients and families in their homes, assisted living, and skilled nursing facilities across Norwalk, Stamford, and Greenwich, CT. Responsibilities include comprehensive assessments, care coordination, and collaboration with the interdisciplinary team.

The role requires RN licensure in the state, 1–2 years of RN experience with hospice care preferred, and basic iOS familiarity.

Formación

• Graduate of an accredited RN program.
• Registered Nurse licensed in the state of employment.
• At least 1–2 years of RN experience is a MUST.
• Hospice care experience strongly desired.
• Basic knowledge of iOS operating system.

Responsabilidades

• Provides skilled nursing care including medications and treatments per physician plan for home patients.
• Performs comprehensive assessments and coordinates the plan of care with disciplines and caregivers.
• Observes and reports patient status to nursing supervisor and physician.
• Participates in weekly Interdisciplinary Team Meetings with updates on patient status.
