Constellation Health Services seeks a compassionate Per Diem/PRN Registered Nurse to provide skilled nursing care for hospice patients in homes, assisted living facilities, and skilled nursing facilities in Millinocket, ME.

The role includes assessing patients, coordinating care with the interdisciplinary team, documenting accurately, and supporting families. Weekend/holiday coverage is required; competitive benefits are offered.

Qualifications

• Graduate of an accredited RN program.
• RN licensed to practice in the state of employment.
• 1-2 years RN experience in hospice or home health preferred.

Responsibilities

• Provides skilled nursing care in patients' homes, assisted living facilities, and SNFs as prescribed.
• Performs comprehensive assessments and coordinates plan of care with interdisciplinary team.
• Monitors patient status and reports changes to nursing supervisor and physician.
• Documents care using the agency's electronic system in a timely manner.
• Participates in weekly interdisciplinary team meetings; supports hospice aides and family caregivers.
• Weekend and holiday coverage as needed.
