Job summary

Trinity Hospice is seeking an experienced RN Case Manager to deliver exceptional hospice care in Joplin, MO. You will assess patients and families, develop individualized care plans, and coordinate nursing interventions to maintain comfort and quality of life.

Qualifications

• Current RN license in the state where you will be working.
• 1+ year experience as an RN Case Manager or similar role in hospice preferred.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Perform comprehensive nursing assessments to identify physical, psychosocial, and emotional needs.
• Develop and evaluate patient-centered care plans focused on comfort and quality of life.
• Provide skilled nursing interventions including pain and symptom management, meds, wound care, and monitoring.
• Coordinate care as the primary RN case manager with the interdisciplinary team.
• Collaborate with physicians, nurse practitioners, social workers, chaplains, and aides to ensure seamless care.

A Day in the Life:

• Perform comprehensive nursing assessments of patients and families to identify physical, psychosocial, and emotional needs
• Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort, symptom control, and quality of life
• Provide skilled nursing interventions, including pain and symptom management, medication administration, wound care, and ongoing clinical monitoring
• Utilize critical thinking and clinical judgment to recognize changes in condition and intervene appropriately
• Serve as the primary RN case manager, coordinating care across the interdisciplinary team
• Collaborate closely with physicians, nurse practitioners, social workers, chaplains, and hospice aides to ensure seamless care delivery
• Facilitate communication among team members to support timely and effective care planning
• Maintain accurate, timely, and thorough nursing documentation in accordance with regulatory and organizational standards (Wellsky/Consolo)
• Participate actively in interdisciplinary team (IDT) meetings, care conferences, and ongoing care planning

Qualifications:

• Current Registered Nurse license in the state which you will be working in.
• One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred.
• Must have a current Driver's license and dependable transportation.
