Job summary

Three Oaks Hospice is seeking a Registered Nurse for PRN positions serving Lexington and Newberry areas. You will administer skilled nursing care in patients' homes, coordinate with the interdisciplinary team, and ensure high-quality end-of-life care.

The ideal candidate has an Associate or Bachelor’s degree in Nursing, RN license, CPR, and at least two years of clinical RN experience; hospice/home health experience preferred. Proficiency with computers and MS Office is expected.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice/home health experience preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal, communication, and organizational skills.
• Demonstrate nursing practice with ethical standards and compliance.
• Ability to cope with family emotional stress and diverse lifestyles.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs.
• Implement the Plan of Care (POC) and revise as necessary.
• Educate patients and families about disease, self-care, end-of-life care; provide staff training as needed.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical progress notes showing progress toward goals.
• Coordinate patient/family services with the Interdisciplinary Team (IDT).
• Use case management approaches and refer to other services as needed.
• Inform physicians and staff of changes in patient needs and responses to care.
• Determine scope and frequency of services based on acuity.
• Take on-call responsibilities and provide on-call service as assigned.
• Other duties as assigned.

Position Summary: Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care. Essential Duties

• Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs and ADLs, i.e., risk for grief, cultural/spiritual, verbal /non-verbal.
• Implement the individualized POC and recommend revisions to the plan as necessary.
• Consulting with and educating the patient/family regarding disease process, self-care techniques, end-of-life care, nutrition and dietary needs. Providing training to other staff as needed.
• Initiating appropriate preventive and rehabilitative nursing procedures.
• Preparing clinical and progress notes that demonstrate progress toward established goals.
• Coordinating all patient/family services and prioritization of needs with the members of the IDT.
• Use of case management approach and referring to other services as needed.
• Informing physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
• Determining scope and frequency of services needed based on acuity and patient/family needs.
• Assessing ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
• Evaluating own needs for support and using identified systems to meet the need
• Communicating information using current process and technology available to the organization.
• Applying specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and the patient’s eligibility.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Other duties as assigned.
