Job summary

Elara Caring in Indiana is seeking an experienced Registered Nurse to support hospice care pathways, identify eligible patients, and coordinate admission and documentation with care teams.

You will assess holistic patient needs, discuss goals with patients and families, and educate partners on eligibility and transitions. Elara offers comprehensive benefits and a mileage program.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 3+ years’ experience in a clinical care setting.
• Experience within Home Health or Hospice is preferred.
