Elara Caring is seeking a Transitional Liaison Registered Nurse to join a team focused on hospice care coordination. The role includes assessing patient needs, guiding transitions, and collaborating with care teams to ensure appropriate service levels and timely referrals.
The position requires an active RN license and 3+ years of clinical experience, preferably in Home Health or Hospice. A hybrid work model is offered with benefits and full support from the Transitional Care Team Lead.
