Job summary

Suncrest Hospice is seeking a Registered Nurse (RN) to provide in-home and facility-based hospice care across our service area. You will work as part of an interdisciplinary team, delivering compassionate medical support and psychosocial assessments to patients and families.

The role requires active RN licensure, the ability to travel, and strong teamwork with nurses, social workers, chaplains, and CNAs. Hospice experience is preferred, and you will manage visits, documentation, and patient

Qualifications

• Current Registered Nurse (RN) licensure in the state(s) of practice.
• Hospice experience preferred.
• Works well within a team with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work independently with phone, email, and meetings with healthcare professionals.

Responsibilities

• Provide care and services for a group of patients, visiting them where they reside.
• Contribute as part of the interdisciplinary team with psych-social assessments and interventions.
• Work in Case Management, Admissions, or Resource roles.
• Maintain schedules, patient communication, and documentation.
