Job summary

Hospice of the Western Reserve, Inc. is seeking a Registered Nurse (RN) for an every-other-weekend role in Mansfield, Ohio.

The position requires providing skilled nursing and personal care to terminally ill patients under physician direction, with collaboration from the interdisciplinary team. You will assess patient needs, manage pain and symptoms, offer education to patients and families, and participate in care conferences and discharge planning.

Qualifications

• Licensed RN in the State of Ohio.
• Current CPR certification.
• Must have and maintain a valid driver’s license with a good driving record.

Responsibilities

• Provide prescribed medical treatment and personal care to terminally ill patients.
• Coordinate plan of care with the transdisciplinary team.
• Monitor pain and symptoms and educate patients/families.
• Maintain and complete hospice care plans and reports.
• Participate in care conferences and case reviews.
• Assist with discharge planning and on-call rotation.
• Communicate with case managers and volunteer staff as needed.
