Job summary

Qualifications

Responsibilities

• Perform initial nursing evaluation visit.
• Continuously assess patient needs and adjust care plans.
• Initiate, coordinate and revise treatment plans with the care team.
• Provide nursing care and perform treatments as ordered by the physician.
• Participate in Quality Assurance/Quality Improvement processes.
• Prepare accurate and timely reports and documentation.
• Adhere to all nursing practice standards and guidelines.
