Job summary

The Care Team Hospice Care is seeking an experienced Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings. You will collaborate closely with the interdisciplinary team to ensure high-quality, patient-centered care in Wayne County, MI.

Schedule is Monday–Friday, 8:00 AM–4:30 PM with potential on-call rotation for additional pay.

Qualifications

• Active RN license and state licensure in good standing.
• Strong clinical assessment and care coordination skills.
• Ability to document care plans accurately in EMR and collaborate with interdisciplinary team.
• Experience in hospice or home health preferred.

Responsibilities

• Conduct comprehensive patient assessments and develop plans of care.
• Provide skilled nursing care per hospice standards and orders.
• Coordinate with physicians, caregivers, and the interdisciplinary team.
• Educate patients and families on symptom management and end-of-life care.
• Document assessments and interventions in EMR HCHB; participate in care planning.
• Support after-hours needs as part of an on-call rotation when applicable.
