Job summary

The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinate end-of-life support for patients at home and in facility settings in Wayne County, MI.

You will work with an interdisciplinary team, follow hospice standards, document in HCHB, and participate in on-call rotation with compensation.

Qualifications

• Active RN license required.
• Hospice or home health experience preferred.
• Ability to work independently in the field.
• Strong communication and documentation abilities.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care in accordance with hospice standards and physician orders.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Educate patients and families on symptom management and end-of-life care.
• Document assessments, interventions, and care plans in the EMR HCHB.
• Participate in interdisciplinary team meetings and care planning.
• Support after-hours patient needs as part of an on-call rotation.
