Job summary

Ascend Hospice & Palliative Care PA in York, PA seeks an RN Case Manager to join our compassionate team. You will perform initial assessments, care planning, and on-call coverage to ensure continuous, well-coordinated hospice care for patients and families.

We offer a $7,500 sign-on bonus, a Monday–Friday schedule, and dedicated opportunities for growth with competitive benefits. Must hold valid RN license and be willing to travel in the York/Lancaster area.

Qualifications

• Must have State Certified RN license.
• 2 years' nursing experience; hospice experience preferred.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial patient and family assessments to determine hospice care needs.
• Regularly re-evaluate patient nursing needs and adjust care plans.
• Collaborate with healthcare team to coordinate continuous care.
• Counsel patient and family in end-of-life needs.
• Initiate preventive and palliative nursing procedures.
• Communicate with physician about patient needs and changes.
• Supervise Hospice Aides.

Now Hiring –RN Case Manager –York, PA (York/Lancaster Area)

Sign on Bonus: $7,500

Hours: M-F 8AM-5:30PM

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs

• Regularly re-evaluates the patients nursing needs

• Use assessment data to determine care plan needs

• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated

• Counsels the patient and family in meeting end of life needs

• Initiates appropriate preventative and palliative nursing procedures

• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition

• Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license

• 2 years' experience as a professional nurse, hospice experience preferred

• Must have a valid Driver's license and reliable transportation

• Must be willing to travel

• Strong verbal and written communication skills.
