Job summary

Affinity Hospice is seeking a registered nurse/case manager to lead the interdisciplinary team in delivering hospice services in patients’ homes and communities. The role emphasizes community health and coordinating care to meet patients and families’ needs within their homes and local areas.

You will collaborate with physicians and staff, develop and document the patient plan of care, and ensure high-quality, compliant care.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse.
• Current CPR certification.
• Must be a licensed driver with an automobile that is insured and in good working order.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect both staff and patient.
• Assure continuity of quality patient care delivered with appropriate documentation.
• Monitor assigned cases to ensure compliance with third-party payer requirements.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients and families.
