The Registered Nurse/Case Manager plans, organizes, directs, and provides hospice care with emphasis on community health education, drawing on community resources to meet patient and family needs at home and in the community. This full-time on-call position requires nursing expertise and a commitment to high-quality care.

Qualifications

• Graduate of an accredited Nursing program (Diploma/ADN/BSN).
• Current State RN license.
• Current CPR certification.
• Minimum 2 years RN experience in clinical care, home health or hospice.
• Licensed driver with insured vehicle in good standing.
• Excellent observation, verbal and written communication, problem-solving, and math skills.
• Able to lift, position and transfer patients; lift supplies and equipment.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement/develop/document the patient plan of care.
• Provide care utilizing infection control measures that protect staff and patient (OSHA).
• Assure continuity of quality patient care with proper documentation.
• Monitor assigned cases to ensure third-party payer compliance.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties as assigned by management to support department goals.
