Job summary

Bloom Healthcare is seeking a Hospice Registered Nurse Case Manager to provide and coordinate home-based hospice care in the Lakewood, Colorado area. The RNCM will educate patients and families, coordinate the interdisciplinary team, and ensure symptom management and comfort at the end of life.

Qualified candidates hold a CO RN license, have 1+ year in med-surg/ER/ICU, and bring strong assessment and communication skills.

• Accredited nursing school graduate.
• Valid CO RN license.
• At least one year in med-surg, emergency or critical care nursing.
• Strong clinical assessment capability.

Job Summary

RNCM's at Bloom hospice have the primary responsibility to provide and coordinate hospice care for our patients. They providecare and help steerthe interdisciplinary hospice team ensuring that ourpatients' physical, emotional and spiritual needs are met. RNCM's educate patients and families about the disease process, help manage and control symptoms, and offer support and strength during difficult times.Care deliverytakes placein a community or where the patient resides.

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $90,000 - $105,000annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
