Job summary

Bloom Healthcare is seeking an experienced Hospice On Call RN to provide and coordinate after-hours hospice care in patients' homes. You will work with the interdisciplinary team to meet physical, emotional, and spiritual needs, educate families, and document care provided.

The role requires a CO RN license, nursing degree, and at least one year in medical/surgical or critical care settings. Home-based care and after-hours scheduling are involved.

Qualifications

• Graduate of accredited school of nursing.
• Current CO RN licensure in good standing.
• Minimum one year of medical, surgical or critical care experience.

Responsibilities

Position Profile:

On call RN's at Bloom hospice have the primary responsibility to provide and coordinate hospice care for our patients and families during after-hours. They give care and collaborate withthe interdisciplinary hospice team to make sure patients' physical, emotional, and spiritual needs are met. On call RN's educate patients and families about the disease process, help manage and control symptoms and offer support and strength during difficult times.Care delivery primarily takes place in the patient's home or in a communitywhere the patient resides and calls home.

Hospice On Call RN Responsibilities:

• Provide hospice patient care after-hours as needs arise for our patients.
• Provides effective pain/symptom assessment and management.
• Provides effective patient/family/caregiverteaching.
• Manages end-of-life ethical issues effectively.
• Creates timely and accurate documentation.
• Utilizes resources effectively and efficiently.
• Adheres to scope of practice,and continuously improves processes and services.
• Maintains and enhances professional skills.
• Adheres to high standards of personal and professional conduct.

Hospice On Call RN Minimum Qualifications:

• Graduate of accredited school of nursing.
• Current CO RNlicensure in good standing.
• Minimum one year of recent experience in medical, surgical or critical care as a professional nurse.
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Solution driven, creative and resourceful problem solving skills.
• Proficient computer skills.

Hospice On Call RN Preferred Qualifications:

• Previous hospice or oncology experience.
• Knowledge of the death/dying and bereavement process.
• CHPN certification.

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $75,000 - $100,000 annually with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
