Qualifications

• Holds a valid Colorado State Nursing License as a Registered Nurse.
• Minimum three years of clinical nursing experience in end-of-life care.
• Prior leadership or supervisory experience required.
• Experience with CHAP, ACHC or JCAHO required.
• Self-directed and able to work autonomously.
• Strong clinical coordination and communication skills.

Responsibilities

• Conduct rigorous clinical assessments of cases including prognosis and pharmacotherapy.
• Provide structured guidance to clinicians to uphold quality standards.
• Collaborate with staff for goal setting and care planning.
• Oversee or assist in staff performance evaluations.
• Lead clinical case conferences and promote interdisciplinary collaboration.
• Develop and refine clinical protocols and organizational policies.
• Maintain professional standards and ethics; advocate for patient-centered care.
• Engage in quality assurance and performance improvement initiatives.
• Pursue ongoing professional development through workshops and networks.
• Undertake additional tasks as directed by the Director of Clinical Services.

Position Profile:

This rolewill work with hospice clients to provide personal engagement and enhance quality of life.

Clinical Qualifications:

• Holds a valid Colorado State Nursing License as a Registered Nurse.
• Has a minimum of three years of clinical nursing experience, specializing in end-of-life care.
• Prior experience in clinical leadership or supervisory roles REQUIRED.
• Prior experience with CHAP, ACHC or JCAHO REQUIRED.
• Capable of self-direction and autonomous work.
• Skilled in clinical coordination and communication.

Clinical Job Function/Responsibilities:

• Advocates for a customer-centric approach within the clinical team.
• Engages in clinical quality assurance and performance improvement, providing leadership and expertise.
• Commits to ongoing professional development through clinical workshops and participation in clinical networks or societies.
• Undertakes additional tasks as assigned by the Director of Clinical Services.
