Bloom Healthcare, a Colorado leader in population health management, home‑based primary care, and hospice, is seeking a hospice nurse to join our expanding Hospice team as an Registered Nurse. Nurses at Bloom hospice have the primary responsibility to provide and coordinate hospice care for our patients. They provide care and help steer the interdisciplinary hospice team ensuring that our patients’ physical, emotional and spiritual needs are met. Nurses educate patients and families about the disease process, help manage and control symptoms, and offer support and strength during difficult times. Care delivery takes place in a community or where the patient resides.
