Elara Caring is seeking a compassionate Hospice Registered Nurse Case Manager in East Lansing, MI. This full-time role requires a licensed RN with at least 1 year of clinical nursing experience.

You will assess patient and family needs across physical, psychological, social, and spiritual dimensions and coordinate with the care team to implement the interdisciplinary plan of care. You will educate patients and staff on hospice care, pain and symptom management, and provide bereavement support

Formación

• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license or reliable transportation to travel between patient homes and assigned work locations.

Responsabilidades

• Monitor patient condition and communicate changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
