Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to join our Defiance County team. This full-time role focuses on assessing patient needs, coordinating interdisciplinary care, and educating families within home and facility settings.

The RN will monitor changes, communicate with the care team, and provide bereavement support while traveling to patient homes in the Defiance area. Prior hospice experience is preferred; a valid RN license and a nursing degree are required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate/ Diploma/ Associate/ BSN).
• At least 1 year of clinical nursing experience.
• Ability to lift up to 50 pounds and travel to patient homes.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate patients, families, caregivers, and staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
