VNA Health Group invites a Staff Nurse Hospice to join its Essex County team. You will assess patient needs, manage symptoms, and develop individualized care plans to support patients and families at end of life.

You will collaborate with VNA clinicians, document updates in EMR, and provide education and guidance to families as care goals evolve. A two-week orientation will introduce hospice practices and safety protocols.

Formación

• RN with Bachelor's degree in Nursing.
• Certification in Hospice Palliative Care Nurse preferred.
• Certification in Critical Care preferred.
• 1+ year of supervised clinical experience as a registered nurse.
• Excellent communication, computer, organizational and interpersonal skills.

Responsibilities

How You’ll Move Hospice Care Forward:

• Complete a two-week orientation focused on hospice care practices, EMR systems, and safety protocols.
• Assess patients with advanced illnesses and create individualized hospice care plans.
• Provide education, emotional support, and guidance to patients and families as they navigate the end-of-life journey.
• Collaborate closely with VNA clinicians and external providers to ensure seamless, compassionate care.
• Document patient status updates and adjust care plans as needs and goals evolve.

Qualifications

• RNwith Bachelor’s degree in Nursing
• Certificationin Hospice Palliative Care Nurse and Critical Care and Critical Careskills preferred
• 1+year of supervised clinical experience as a registered nurse
• Excellentcommunication skills
• StrongComputer skills
• Excellentorganizational and interpersonal skills

Working Conditions/Physical Demand

Working indoors/outdoors with clinical equipment and use of phone and computer; car and valid NJ Driver’s license
