As a Hospice RN / RN Case Manager, You Will

1. Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control and professional management of each patient.
2. Act as the primary liaison between physician, patient/family, and hospice team.
3. Provide direct patient care services to hospice patients and supportive care to the patient’s family, fostering trusting and compassionate relationships.
4. Empower caregivers through education on care for their loved ones.
5. Manage and collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to provide comfort, peace, and dignity.
6. Evaluate patient and family needs and assist in developing and executing care plans.
7. Maintain all clinical records for assigned patients.
8. Participate in quality assessment and performance improvement programs.

About You

1. Compassionate, kind, and empathetic to patients and families.
2. Comfortable providing care in the patient’s home.
3. Flexible and able to support others during crises.
4. RN license in the state of employment (or eligibility).
5. At least 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
6. Additional experience in hospital settings, ER, critical care, or related fields is advantageous.
7. Current driver’s license and automobile insurance.
8. Current CPR certification.
